Two versions of the forms are available for your convenience -

Online version:

	Use this form if you wish to respond to an application. [online version]
	Application in a Proceeding (Form 4 Rule 6)
	Use this form for any application in a proceeding that has already been started. [online version]
	Additional Information and Statutory Declaration
	This form was introduced by Practice Direction No 1 of 2001. It is used by applicants for mining leases or for additional surface areas of land.
